WHAT ARE DENTAL IMPLANTS REALLY?

Dental implants are the most long-term and most adequate substitute for a tooth root.

IMPLANT INSERTION

The first step in the dental implant insertion procedure is making an individual treatment plan.

An obligatory part of the intervention planning is a 3D X-Ray that shows your dentist the precise bone size, and also the width and height of the ridge (upper or lower jaw) as well as the proximity of different anatomic structures. These are: mandibular nerve, mental foramen, maxillary sinus, nose cavity and other.

Based on the clinical examination and a 3D X-Ray it is determined what implant would suit best and what prosthodontics could be attached on such an implant.

IMPLANT INSERTION PROCEDURE IS PERFORMED IN TWO STAGES:

STAGE 1

The first stage of implant insertion is the surgical intervention itself. Although the very procedure is considered to be a surgical intervention, which may sound scary, today it is a short routine procedure. Implant insertion is performed in local anaesthesia and it is completely painless.

The post-operational stage is mostly the same as with any surgical intervention. The pain experienced after surgery is controlled by analgetics and temporary mild swelling is treated with cold compresses (packs). In approximately 95% of cases patients are fully able to get back to their everyday activities after 24 hours.

 

STAGE 2

After implant insertion, osseointegration (the implant grows into the bone) occurs. The osseointegration in the upper jaw can last to up to 4-6 months, whereas in the lower jaw it will last approximately 3 months.

The second phase also understands opening the implants in local anaesthesia and taking a mould for a definitive prosthodontics. Whether a fixed prosthodontics (not removable) or a mobile one will be attached depends on the number of implants as well as the remaining teeth in your jaw.

When we say a fixed prosthodontics, we mean crowns and bridges which are cemented on implants and are not removable. However, today we can have fixed prosthodontics which are not removable in themselves but they can be removed by a dentist. Those are fixed-detachable crowns and bridges. This enables a detailed cleaning of all spaces in the mouth, which patients can not perform on their own. After cleaning, the prosthodontics is re-attached in the patient’s mouth.

 

FIXED OR MOBILE PROSTHODONTICS

Apart from the number of implants, the implant positioning factor also influences the type of prosthodintics (fixed or mobile). Sometimes with toothless patients only four or six implants so-called all on 4 or all on 6) in certain positions are enough to make a prosthodontics that will be utterly stable to hold the chewing force.

If, however, the mouth does not contain enough bone to insert more than two implants, prostheses are a definitive prosthodontic solution to the problem. They are attached to the implants with special connections - attachments - that are located inside the prosthesis. This kind of prostheses are extremely stable and reliable.
 

CAN ANYONE HAVE A DENTAL IMPLANT INSERTED

Implant insertion success rate is of 97%.

What is imperative for implant insertion is good general and oral health of the patient.

Counter-indications as such do not exist, except with radiation and chemotherapy patients. Caution is needed with patients whose immune system is completely compromised, those who suffer from renal insufficiency or have had a heart attack recently. It is also very important to stress that uncontrolled diabetes or chronic auto-immune diseases are a relative counter-indication. Taking certain medication can also influence osseointegration (for instance, corticosterioids or immunosuppressants).

In the aforementioned cases a more detailed analysis is done, after which a decision as well as a compromise between objective possibilities and the patient’s desire regarding implants are made.

 

DENTAL IMPLANTS MAINTENANCE

What is extremely important after implant insertion and later on - after the attachment of some kind of prothodontics is oral hygiene.

All the articles in which you have read that implants: do not have to be maintained, have a life-long guarantee etc. are simply not true.

Quite the contrary, it is of extreme importance to maintain your oral hygiene and use inter-dental tooth brushes apart from your regular toothbrush. They reach spaces which are not possible to brush with your regular toothbrush and thus clean all spaces from dental plaque (calculus). It is also advisable to use a mouthwash as well as to pay regular visits to your dentist, for a check-up.

Apart form the all afore-stated, there are devices in the market (Water pick) which help significantly with the correct oral hygiene maintenance.
